At TCHC, our philosophy is that seniors are better cared for when one-on-one care is provided. We provide a holistic approach that encompasses a community-based approach. We work with our clients, families, and their primary care networks such as pharmacy services, hospital services, and other social services.

Why stay at home?

Living in a long-term care facility can carry a higher monthly cost compared to staying in your own home. Ageing in your own space means you get to continue living a more independent lifestyle.

Your home has all of your meaningful belongings and precious memories. Why leave if you don’t have to? Ageing in place means you get to choose what activities you want to do, have visitors as you please, and eat what and when you want.

Long-term Care Facilities often have smaller living spaces or shared accommodations. Generally, independence is limited as activities have to be scheduled around the activities of other residents as well. Furthermore, part of your care provider costs can be claimed in your income taxes thus reducing overall cost.

For loved ones in care facilities

If your loved one is in a long-term care facility, they can greatly benefit from the undivided care of TCHC staff. With our services, your loved ones are guaranteed to receive our full-time attention, whereas long-term care facilities only provide an average of 1.5 hours of undivided attention per day.

Respite

It can be difficult as a caregiver to provide care for your loved ones. We also understand that, as a primary caregiver, you may not always be available to support your family member. Personalized plans

All of our plans are personalized to our clients. We do not believe there is a one size fits all approach when it comes to care. We provide services to a wide range of clients from those who may need companionship to those with chronic diseases and palliative care. We offer the option of Live-In caregivers for short or long-term placements as well as respite and overnight care.

Here at TCHC, we believe that all people, regardless of their physical or mental disability, should be treated with dignity and have an opportunity to exercise their independence. This is especially the case for clients with mobility and cognitive disabilities or survivors with chronic conditions.

We offer several home support services (e.g. cooking, cleaning, laundry, grocery shopping) and personal care services (e.g. toileting, bathing, and clothing) to assist our clients with expanding and sustaining their independence.

Furthermore, we can assist with community support services. This includes offering companionship and arranging transportation to various appointments or social outings that encourage healthy emotional well-being. These support services are tailored specifically for every client’s care plan and our staff are trained to work with you in monitoring and achieving your goals or adjusting them as needed.
